Vermont Commissioner of Health, Harry Chen, MD, Visits Blueprint Team at Copley Hospital | Monday, August 29, 2011 Morrisville, VT - Harry Chen, MD, the Vermont Commissioner of Health, recently visited the Integrated Health Services Team of the Blueprint for Health for the Copley Service Area. Dr. Chen learned how Community Health Services of Lamoille Valley, Northern Counties Health Care, Copley Hospital, Lamoille Home Health & Hospice, The Manor, the Vermont Department of Health and the Vermont Department of Health Access are integrating prevention and public health services. As part of the Blueprint program, the group is creating Community Health Teams (CHTs) that will be housed in primary care practices. The CHTs will help patients with chronic conditions better manage their conditions, answering questions, scheduling follow up appointments and generally serving as each patient’s case manager. Recently hospitalized patients may also be referred to a CHT to ensure they receive the follow-up care prescribed. The Blueprint program is one of Vermont’s healthcare reform initiatives that is being closely watched on a national level.
